Circulating Endothelial Cells

Abbreviation: CEC, CECs

Type: Cellular biomarker / endothelial injury marker / vascular and angiogenesis biomarker

Function: Circulating endothelial cells are mature endothelial cells that detach from the vascular endothelium and enter the peripheral blood. They are normally present at very low concentrations, and increased numbers can indicate endothelial injury, vascular dysfunction, inflammation, or increased vascular remodeling.

Cancer: ↑ CEC levels can occur in cancer and may reflect increased endothelial turnover, tumor-associated angiogenesis, vascular injury, or treatment effects. CECs have been investigated as circulating biomarkers of tumor vascular activity and response to antiangiogenic therapies.

Favorable Direction in Cancer: Context-dependent. ↓ elevated CEC levels may indicate reduced tumor-associated vascular turnover or recovery from endothelial injury. However, some anticancer or antiangiogenic therapies can transiently ↑ CECs by damaging tumor vasculature, so treatment-associated changes should be interpreted according to timing and mechanism.

Interpretation Note: Circulating endothelial cells should be distinguished from circulating endothelial progenitor cells (CEPs/EPCs). CECs are predominantly mature endothelial cells shed from existing vessels, whereas endothelial progenitor populations are less differentiated cells associated with vascular repair and neovascularization.

Measurement Note: CEC definitions vary between studies because different flow-cytometry and immunomagnetic marker combinations are used. Reported CEC counts should therefore be interpreted using the specific phenotype and assay defined by the study.
