Ketosis — Metabolic state characterized by increased hepatic production and systemic utilization of ketone bodies, primarily beta-hydroxybutyrate (BHB), acetoacetate, and acetone. Ketosis commonly occurs during fasting, carbohydrate restriction, ketogenic diets, prolonged exercise, or other conditions that increase fatty-acid oxidation and reduce reliance on glucose.

Typical modulation in Alzheimer’s disease: ↑ nutritional ketosis may be beneficial in some contexts by increasing cerebral availability of ketone bodies as an alternative energy substrate when brain glucose utilization is impaired. Ketogenic interventions and exogenous ketones have therefore been investigated in mild cognitive impairment and Alzheimer’s disease.

Typical modulation in cancer: Context-dependent. Ketosis can alter glucose availability, insulin signaling, fatty-acid oxidation, ketone utilization, and tumor metabolism, but effects vary substantially by tumor type and metabolic phenotype. A universal anticancer direction should not be assigned.

Typical modulation in metabolic disease: Nutritional ketosis may be associated with increased fatty-acid oxidation, reduced circulating glucose and insulin, and altered substrate utilization. This should be distinguished from pathological ketoacidosis.
