CTSS — Cathepsin S

Primary role: Cysteine protease active at neutral pH; mediator of antigen processing and immune modulation.

Cancer relevance:
CTSS functions both lysosomally and extracellularly, particularly in tumor cells, antigen-presenting cells, and stromal compartments. It alters MHC class II antigen processing, promotes immune evasion, and contributes to angiogenesis and invasion. Unlike other cathepsins, CTSS remains active outside acidic lysosomes, amplifying its effects in the tumor microenvironment.

Net effect in established cancer:
Predominantly pro-tumor, through immune suppression, invasion, and resistance to immunotherapy.
