Platelet-activating factor (PAF) is a potent bioactive phospholipid mediator involved in inflammation, platelet activation, vascular permeability, leukocyte recruitment, bronchoconstriction and immune signalling. PAF acts primarily through the platelet-activating factor receptor (PTAFR / PAFR), a G-protein-coupled receptor. Excessive PAF signalling can contribute to inflammatory and vascular injury and has also been implicated in tumour-cell proliferation, migration, angiogenesis and metastasis in some cancers. In the nervous system, excessive PAF signalling can promote neuroinflammation, oxidative stress and excitotoxic neuronal injury, although physiological PAF also participates in synaptic signalling. Therefore, pathological PAF activity is generally considered up in inflammatory disease contexts, while the desired modulation is typically down or PAF-receptor antagonism. Ginkgolides, particularly ginkgolide B, are well-characterized antagonists of the PAF receptor and contribute to the pharmacological activity of standardized Ginkgo biloba extracts such as EGb 761.

7228- EGb 761,    Current Perspectives on the Beneficial Role of Ginkgo biloba in Neurological and Cerebrovascular Disorders
- Review, AD, NA
*Dose↝, A standardized formulation, EGb 761®, also sold as Tanakan® or Tebonin®, was created to normalize the constituents to assure reliable and consistent drug performance and the absence of ginkgolic acid, a known allergen naturally found in Ginkgo
*PAF↑, Ginkgolides have been clinically shown to act as platelet-activating factor (PAF) antagonists, inhibiting platelet aggregation and promoting increased blood flow.
*AntiAg↑,
*BloodF↑,
*Inflam↓, Additionally, BB(bilobalide) has shown anti-inflammatory properties and neuroprotection in preclinical models of stroke16 and AD
*neuroP↑,
*Stroke↓, Alternatively, promising results have been observed that support administration of EGb 761 soon after the onset of ischemia and during the recovery period.
*Dose↝, The standard clinical dose of EGb 761 is 120 mg (~1.7 mg/kg) once or twice daily; thus, a standard dose will contain ~3–4 mg ginkgolides A, B, and C, 3–4 mg BB, and 29 mg flavonoids.
*Sepsis↓, Patients treated with antimicrobials plus GB showed a significant drop in mortality as compared to patients treated with antimicrobials and placebo; this finding led to the development of other PAF antagonists for the treatment of sepsis
*memory↑, when administered daily, EGb 761 (240 mg daily) may improve some aspects of memory better than others

7268- GGB,    Ginkgolide B inhibits the neurotoxicity of prions or amyloid-beta1-42
- in-vitro, AD, NA
*PGE2↓, The ginkgolides reduced the production of prostaglandin E2 in response to amyloid-β1-42 or sPrP106
*Casp3↓, ginkgolides reduced caspase-3 responses to amyloid-β1-42 or sPrP106, and in co-culture studies the ginkgolides reduced the killing of amyloid-β1-42 or sPrP106 damaged neurons by microglia.
*neuroP↑, Nanomolar concentrations of the ginkgolides protect neurons against the otherwise toxic effects of amyloid-β1-42 or sPrP106.
*PAF↑, The protective effect of the ginkgolides in vitro correlated with their ability to inhibit PAF
