Function: HTRA2/Omi is a mitochondrial serine protease involved in mitochondrial protein quality control, cellular stress responses, autophagy, and apoptosis. Under mitochondrial stress, Omi can be released into the cytosol where it antagonizes inhibitor-of-apoptosis proteins (IAPs) and promotes caspase-dependent and caspase-independent cell death.

Cancer: ↕ Context-dependent. Cytosolic HTRA2/Omi can promote tumor-cell apoptosis by antagonizing IAP-mediated survival and activating cell-death pathways, supporting a tumor-suppressive role. However, mitochondrial HTRA2 also maintains mitochondrial homeostasis and stress tolerance, so its overall effect depends on cellular localization, protease activity, and tumor context.

Alzheimer's Disease: ↑ Increased HTRA2/Omi protease activity and increased active/cytosolic Omi have been reported in Alzheimer's disease brain. This altered activity may contribute to mitochondrial dysfunction, cellular stress, and neuronal apoptosis, although total HTRA2 mRNA expression may remain relatively unchanged.
