Abbreviation: 2/16-OHE1 ratio, 2-OHE1/16α-OHE1, EMR

Type: Estrogen metabolism biomarker / metabolite ratio

Function: Represents the relative metabolism of estrone through the 2-hydroxylation versus 16α-hydroxylation pathways. 2-Hydroxyestrone has relatively weak estrogenic activity, whereas 16α-hydroxyestrone has greater estrogenic and potentially genotoxic/proliferative activity.

Cancer: ↓ A lower 2-OHE1/16α-OHE1 ratio has historically been proposed to represent a less favorable estrogen-metabolism profile and potentially greater breast-cancer risk, while an increased ratio has been considered potentially favorable. However, prospective studies and systematic reviews have found inconsistent associations, and the ratio is not considered a validated predictor of breast-cancer risk.

Direction: ↑ Traditionally considered favorable, but clinical significance remains uncertain.
