Function: β-Hydroxybutyrate is a major circulating ketone body produced primarily by the liver during fasting, carbohydrate restriction, prolonged exercise, or ketogenic metabolism. It serves as an alternative mitochondrial energy substrate and also functions as a signaling metabolite that can regulate NLRP3 inflammasome activity, histone deacetylase activity, oxidative stress, mitochondrial function, gene expression, and inflammatory responses.

Cancer: ↕ Context-dependent. β-Hydroxybutyrate can inhibit inflammatory and growth-related signaling and has demonstrated anti-proliferative and anti-migratory effects in some experimental cancer models. However, metabolic utilization of ketone bodies varies among cancers, and some tumor cells can use β-hydroxybutyrate as an energy or biosynthetic substrate.

Alzheimer's Disease: ↑ Generally neuroprotective. β-Hydroxybutyrate provides an alternative energy substrate when cerebral glucose metabolism is impaired and can inhibit NLRP3 inflammasome activation, reduce microglial neuroinflammation, improve mitochondrial function, and reduce oxidative stress. Experimental studies support beneficial effects on cognition and Alzheimer's-related pathology.
