| 5α-Reductase (Steroid 5-Alpha Reductase; SRD5A) — NADPH-dependent steroid-converting enzyme family that catalyzes conversion of testosterone to dihydrotestosterone (DHT), a more potent androgen receptor agonist. The principal human isoenzymes are SRD5A1, SRD5A2, and SRD5A3. BPH relevance: ↑ 5α-reductase activity, particularly SRD5A2 in prostate tissue, increases conversion of testosterone to DHT and promotes androgen-dependent prostate growth. Inhibition of 5α-reductase lowers DHT, reduces prostate volume, improves lower urinary tract symptoms, and reduces risk of urinary retention and BPH-related surgery. Typical modulation in cancer: Context-dependent but frequently tumor-supportive in prostate cancer through maintenance of intratumoral DHT and androgen receptor signaling. SRD5A1 and SRD5A3 may become relatively more important in advanced or castration-resistant prostate cancer, whereas SRD5A2 expression can decrease during progression. Typical modulation in Alzheimer’s disease: No established core Alzheimer’s disease direction. 5α-reductase enzymes also participate in neurosteroid metabolism, but their role in AD is complex and not suitable for a universal ↑ or ↓ designation. |
