| Apparent Diffusion Coefficient (ADC) — Quantitative imaging biomarker derived from diffusion-weighted magnetic resonance imaging (DWI) that measures the apparent mobility of water molecules within tissue, typically expressed in mm²/s. Typical modulation in cancer: ↓ ADC is commonly associated with restricted water diffusion and increased tumor cellularity and may correlate with higher tumor grade or viable tumor burden. ↑ ADC after effective anticancer treatment commonly reflects reduced cellularity, loss of cell membrane integrity, apoptosis, necrosis, or other treatment-induced tissue damage. Treatment-response interpretation: ADC ↑ after therapy is frequently a favorable response marker and may occur before measurable tumor shrinkage. However, interpretation is tumor-, tissue-, timing-, and protocol-dependent, and transient decreases or heterogeneous ADC changes can occur. Typical mechanistic relationship: Tumor cellularity ↑ → extracellular water mobility ↓ → ADC ↓. Effective therapy → cell death / membrane disruption / cellularity ↓ → water mobility ↑ → ADC ↑. |
